Any patient that has a diagnosis of primary diabetes at any time during 2026.
During your visit with provider or diabetic educator, ask about joining the Diabetic Wellness Initiative. You will be given a card small enough to put in wallet or purse that lists 6 screenings (3 can be performed during any visit with a provider and 3 are lab tests) you should have done every year to help prevent complications from your diabetes.

January 2026-December 2026. You will be given credit for any screenings completed already in 2026.
